Integrity Pharmacy chooses MedTek21 to Power Innovative Personalized Medication Management Program for Home Health and Home Care Patients

Enhances Integrity’s leading full-service pharmacy program for home health and home care with benefits to patient safety, outcomes and quality of life

Integrity Pharmacy, a recognized leader in home health and home care pharmacy services, today announced a partnership with MedTek21 to bring continuous personalized medication risk management to all Integrity Pharmacy patients. MedTek21’s software helps Integrity proactively identify potential medication risks, ensuring a safer and more effective medication regimen for entire patient populations.

Reactions to prescription drugs are a major cause of falls, hospital admissions, emergency room visits and other negative outcomes in home care and home health patients. Patients with certain genetic variations face potential danger from many widely prescribed drugs including some common antipsychotics, anticoagulants and pain medications. Through the use of MedTek21’s continuous drug-to-gene monitoring platform, these risks can be both proactively identified and mitigated, leading to reduced risks of drug-related adverse events, better quality of care, quality of life, and lower care costs.
